Your First Visit

Please assist us at the time of your initial visit to the office by providing the following information:

• • Your referral slip and x-rays from your referring dentist (In many cases, your referring dentist will have sent us this information prior to your visit).
  • A comprehensive medical history and list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website and bring it with you for your first appointment).
  • If you have dental insurance, please bring any forms or insurance benefit cards with you to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).

Insurance

We accept most dental insurances including Delta, Cigna, Metlife, and Aetna. Please be aware that your dental insurance may be restricted and designed to cover only a limited portion of treatment. Dental insurance is not and never has been a guideline for quality and effective care. Dr. Rubio will devise a treatment plan that he feels is in the best interest of your health. Please provide the details of your insurance plan prior to your visit so we can ensure that our staff’s able to maximize your benefits.
